We created Coordin8Care to help people get their home health and care services scheduled and keep their health information organized in an easy-to-use mobile interface.

Coordin8Care allows your physician or physician’s office to arrange your home health and care services quickly.

If you’re having surgery, Coordin8Care helps your physician’s team get your post-op care coordinated and confirmed ahead of time, so you can feel confident about your recovery.

Coordin8Care can also be used to view your home health and care services and delivery schedule for chronic conditions such as diabetes, emphysema, chronic obstructive pulmonary disease (COPD) and coronary artery disease (CAD).

Simple to Use

Using Coordin8Care for patients is simple! It’s really just a secure calendar and document folder to keep your home health and care services, deliveries, lab results and medical contacts organized, accessible, and private.

Only one step to get started. Your physician’s coordinator will set you up in the system, input your information and provide you with your login information. All you have to do is download the app from the link below and create your password. That’s it!

Easy Access

Scheduling Appointments

Your schedule of services and deliveries will be initially created with you by the home care providers via phone calls or visits.

After you’ve discussed the schedule that works for you, the home care providers will keep the personalized app calendar updated for you. Nothing for you to do except view the calendar in Coordin8Care on any internet connected device (smart phone, computer or tablet).

Share Calendar with Caregivers

If you would like, you can share your calendar with family and caregivers to help them stay up-to-date on your treatment or recovery plan. You can also download the calendar to your smart phone, personal calendar or print a hard copy.

Appointment Reminders

You’ll receive automatic appointment reminders through Coordin8Care. If you need to change an appointment, you’ll be able to see the contact information of your home care providers so you can quickly get in touch with them.

Lab Results

Lab results will be available for you to download through the Coordin8Care app as well for your personal use or to share with another health care provider.
