Joining the Network

Once you download the app and activate your account, you become eligible to be matched to physicians seeking home health and care services for their patients. Physicians you already work with will be able to select you as a preferred home care provider. Once selected, physicians can quickly assign orders for you to accept or refuse.

Coordinating Patient Care

When you accept orders, you will be able to view patient information, orders, and treatment plan details. After your initial call with the patient to discusss scheduling preferences, you can manage appointment or delivery scheduling through the app.

From the app you can keep track of patient appointments, view treatment plans and protocols, proactively update physicians’ offices on treatment progress, view patient lab results and documentation, verify insurance, and receive reorders for expiring services.

Through the patient portal, patients can easily view their treatment plans and scheduled appointments, reducing the time you spend coordinating home visits or deliveries.

Streamlining Communications

With customized portals that also allow physicians and patients to access the information they need without having to contact you, you can be confident everyone is in the loop and save time with the app’s automated patient appointment reminders.

You’ll be able to update physicians on your progress in live time from any internet connected device.
